I know each city is different, but in general are permits required to install an above ground spa?

All will require an electrical permit. My locale actually required a building permit because they consider it a swimming pool  ::) I got lucky, since I had an open permit on my deck, I just revised that to include the tub. They also required the ETL listing number from the tub.

A quick call to your municipality's building / planning department should answer that question.  In Ottawa, a tub is not covered under the pool section of the building and planning code but is required to be managed just like a pool for insurance purposes.
